Withdrawal/Refund Form

If your refund request adheres to the policy outlined below, the refund will be credited to the original credit card used for payment. For payments made by cash or cheque, a refund cheque will be sent by mail.

• Administrative Fee: A $25 administration fee will be applied to all withdrawals.
• U6-U9 Intro to Hockey: Refunds are available for withdrawals requested before October 30th.
• U11 to U18: Once a player has been drafted to a team, no refunds will be issued—with the exception of AAA/AA players attending second tryouts. In these cases, your spot will be held. If you are selected for a team, a refund will be issued minus the $25 administrative fee, any applicable tryout or late fees, and the 3% credit card processing fee applied to both the original CC payment and the refund to a credit card.
• Tryout Fees: Non-refundable.
• Late Fees: Non-refundable.
• A 3% administrative fee applies to all credit card transactions. This fee is non-refundable and is charged on both the original CC payment and any refunds issued to a credit card.
