Withdrawal/Refund Form

If your refund request adheres to the policy outlined below, the refund will be credited to the original credit card used for payment. For payments made by cash or cheque, a refund cheque will be sent by mail.

• Administrative Fee: A $25 administration fee will be applied to all withdrawals.
• U6-U9 Intro to Hockey: Refunds are available for withdrawals requested before October 30th.
• U11 to U18: No Refunds will be issued once player has been drafted to a team with the exception of AAA/AA players that attend 2nd tryouts.  We will hold your spot, and if you are selected for a team, you will be refunded in full exept for  the Non-Refundable tryout fee and the $25 administrative fee.
• Tryout Fees: Non-refundable.
• Late Fees: Non-refundable.
• Credit Card Fee: The 3% administrative fee for credit card transactions is non-refundable.
