Withdrawal/Refund Form

If your refund request adheres to the policy outlined below, the refund will be credited to the original credit card used for payment. For payments made by cash or cheque, a refund cheque will be sent by mail.

• Administrative Fee: A $25 administration fee will be applied to all withdrawals.
• U7 Intro to Hockey: Refunds are available for withdrawals made before October 31.
• U9 to U18: Refunds are available for withdrawals made before October 15.
• Tryout Fees: Non-refundable.
• Late Fees: Non-refundable.
• Credit Card Fee: The 3% administrative fee for credit card transactions is non-refundable.
