Tier & City League

• Sanction Permit is REQUIRED for exhibition games.

• This ensures insurance coverage for all players and team staff.

• Penalty: No permit = $500 fine + loss of sanctioning privileges.

How to Apply:

• Host team will submit this form.  

  • The Host team and the Visiting team will receive the approval permit with the Sanctioning number.

• Submit the form 7-14 business days before the game.
