Travel / Exhibition / Team Fundraising / Special Event Forms

Managers, it is very important the correct forms are filled out to ensure the insurable activities are covered.

AA & AAA

• Exhibition Games played within Alberta, are not required and exhibition or travel permit.
• You do however need to submit the infor to the league to ensure it is sanctioned:  https://www.aehl.ca/elite-hockye-exhibition-travel-form  
  • Outside of the Province, please follow instructions below.

Tiered & City League 

Travel Permit - required for travel to exhibition / tournament games, see form for further clarification.  

Exhibition Game Permit - applied by the host team to sanction an exhibition game, see form for further clarification. 

Team Fundraising Form - team requires approval before starting any team fundraising, see form for further clarification.

Special Event Form - required for all team gatherings outside of a practice or game, see form for further clarification.
